Abstract
Identification processes for at-risk students in primary schools are pivotal for the implementation of appropriate and timely intervention strategies. With increasingly complex problems facing youth today, it is vital to have identification and referral processes that are effective and efficient. This paper describes an innovative multidisciplinary approach for identification and referral that can be implemented in primary schools.
