Abstract
Home health care is a pivotal service in the continuum of care for older adults. It often represents a fulcrum between restored functioning and institutionalization. The home health care social worker has a unique position in this continuum because she must constantly interface with other professionals throughout the continuum. This interface is perhaps more dynamic for social workers in home health care than in any other place in the care continuum.
