Hemodynamic changes in the periodontium during the treatment of chronic generalized periodontitis using polarized polychromatic light

Abstract

BACKGROUND: the lack of effectiveness of treatment of inflammatory periodontal diseases is a large-scale medical and social problem. Previous studies have established that in the treatment of periodontitis, achieving normalization of vascular reactions and activation of local immunity play an important role. This result is achievable using polarized, polychromatic radiation. The influence of polarized, polychromatic radiation on microhemocirculation in periodontal tissues has not been sufficiently studied.. AIMS:to study microhemodynamic changes in periodontal tissues during the treatment of mild chronic generalized periodontitis using polarized polychromatic light. MATERIALS AND METHODS:a comprehensive dental examination and treatment was carried out in two groups of patients (35 people each aged from 20 to 49 years). In the comparison group, treatment consisted of professional oral hygiene and a course of antimicrobial therapy using Dentamet dental gel. In the main group, antimicrobial therapy was carried out in combination with polarized polychromatic radiation (wavelength 480 - 3400 nm, power - 40 mW/cm2, light energy flux density - 2.4 J/cm2 per minute, polarization - 95%). Hemodynamic studies using laser Doppler flowmetry were carried out before and three, six and 12 months after treatment. . RESULTS:70 patients (32 men and 38 women) aged 20 to 59 years with a diagnosis of chronic generalized periodontitis in the active stage took part in the study and completed it. The patients were divided into two quantitatively equal groups depending on treatment methods. The first group received antimicrobial therapy using Dentamet dental gel. In the second group, after completion of antimicrobial therapy using Dentamet dental gel, phototherapy using PPI was performed. In the first group of patients, improvement in microcirculation parameters was established within a period of up to three months (p 0.05). In the second group of patients, improvement in microcirculation parameters was established within a period of up to one year (p 0.05). CONCLUSIONS:the use of polarized polychromatic light in the treatment of mild chronic generalized periodontitis improves microcirculation in periodontal tissues and creates conditions for achieving greater effectiveness of therapeutic drugs.
