Changes in platelet counts in patients with acute ischemic stroke depending on sensitivity to acetylsalicylic acid therapy

Abstract

The study aims to investigate changes in the thrombocytogram in patients with early recovery from ischemic stroke depending on the sensitivity to acetylsalicylic acid therapy.Material and methods. A clinical and laboratory analysis of 80 patients with acute ischemic stroke (AII), who took acetylsalicylic acid (ASA) at a dose of 75-100 mg per night to prevent recurrent stroke. The control group consisted of 30 healthy individuals who did not take ASA. The functional activity of platelets was determined on a laser aggregometer Biola Aggregation Analyzer and on the results of a platelet cytogram on a hematology analyzer Abacus 30.Results. According to the results of optical aggregometry, 25% of the examined patients did not respond to ASA therapy and were assigned to the first study group; the remaining 75% were aspirin-sensitive and belonged to the second group. Increased platelet aggregation in aspirin-resistant patients was also characterized by changes in platelet counts compared with aspirin-sensitive patients. At the same time, there was an increase in the size of platelets, an increase in platelets distribution width, an increase in the quantitative and percentage number of large-sized platelets.Conclusion. Сomparing the platelet cytogram in aspirin-resistant and aspirin-sensitive patients, there is a significant increase in almost all indicators, which reflects greater functional platelet activity, ie increased aggregation ability and proves the lack of antiplatelet effect in their AS.
