Respiratory diseases generate significant symptom burden, whether they are chronic or acute. Breathlessness is the cardinal symptom of respiratory disease. Rapid changes in breathing for any person raises concerns about imminent death. As such, clinical responsiveness to worsening breathlessness is necessary. Often, as with many symptoms, a reversible cause may be identified. Indeed, the more rapid the onset, the more likely that a precipitant can be identified. Whether or not to try and reverse any identified pathology requires careful clinical judgement and timely discussions with the person with the life-limiting illness and their family. Sepsis is frequently encountered
