592. Impact of Elevated MIC Values on Echinocandin Pharmacokinetic-Pharmacodynamic (PK-PD) Candida glabrata Target Attainment (TA)

Abstract

Abstract Background Given the increasing prevalence of non-albicans Candida species, including C. glabrata and C. auris, which have higher predisposition to develop antifungal resistance (e.g., fks mutations conferring reduced susceptibility to echinocandins), disparities in PK-PD TA of currently approved echinocandins warrant further attention. Rezafungin (RZF) is a novel, next-generation echinocandin in development for treatment and prevention of invasive fungal disease. The completed Phase 3 treatment trial (ReSTORE; NCT03667690) demonstrated non-inferiority of RZF once weekly to caspofungin (CAS) once daily in patients with candidemia and invasive candidiasis. Here we revisit the challenge for echinocandins in achieving TA against C. glabrata with elevated MIC values and build on previous analyses using RZF patient data. Methods Monte Carlo simulations were conducted using published population PK models for anidulafungin (AFG), micafungin (MFG), CAS, and RZF (Bader et al 2018, Rubino and Flanagan 2021). Dosing regimens were as labeled for candidemia for once-daily echinocandins and as studied for RZF (400 mg Week 1, 200 mg once weekly thereafter). Individual free-drug plasma AUC values from time zero to the end of the dosing interval of 24 or 168 h (AUC0-tau) were calculated for each drug for 2 weeks. MIC values corresponding to observed C. glabrata MIC90 and MIC100 values for each drug were derived from 2018-2020 SENTRY surveillance data (Pfaller et al 2020, Carvalhaes et al 2022). Free-drug AUC0-­tau:MIC targets associated with return to baseline colony-forming units (CFU) (stasis) of C. glabrata, derived from neutropenic murine disseminated candidiasis models, were used (Table). Simulated echinocandin free-drug AUC0­-tau:MIC ratio distributions relative to the MIC90 and MIC100 were generated. Results The FDA-approved dosing regimens for CAS, MFG and AFG failed to achieve adequate TA at higher C. glabrata MIC values associated with their respective MIC90 (AFG only) and/or MIC100 (all three). In contrast, the Phase 3 RZF dosing regimen achieved excellent TA at both the MIC90 and MIC100 (Figure). Conclusion Projected enhanced TA for RZF once weekly may result in clinical efficacy in patients infected with less susceptible C. glabrata isolates. Disclosures Christopher M.
