Abstract
TÓM TẮT
Mục tiêu: Mô tả đặc điểm của một số chỉ số huyết học và nồng độ CRP máu cuống rốn ở trẻ sinh non bị nhiễm khuẩn sơ sinh sớm (NKSSS). Tìm mối tương quan của một số chỉ số huyết học và nồng độ CRP giữa máu cuống rốn với máu ngoại vi trong NKSSS ở trẻ non tháng.
Đối tượng, phương pháp: Nghiên cứu mô tả tiến cứu. Lấy mẫu toàn bộ gồm 129 trẻ sơ sinh non tháng có xét nghiệm công thức máu và CRP máu cuống rốn và máu ngoại vi. Hỏi các yếu tố nguy cơ NKSSS, theo dõi lâm sàng và cận lâm sàng trong 48 giờ, sau đó chia thành 2 nhóm: nhóm NKSSS và nhóm không NKSSS để so sánh. Địa điểm nghiên cứu: phòng Nhi sơ sinh, Khoa Phụ sản, Bệnh viện Trường Đại học Y Dược Huế từ tháng 06 năm 2021 đến tháng 06 năm 2022.
Kết quả: Xét nghiệm máu cuống rốn của 77 trẻ NKSSS ghi nhận số lượng bạch cầu trung vị là 9,3 (8,1 - 11,8) G/l, số lượng hồng cầu trung bình là 4,4 ± 0,7 T/l, nồng độ Hb trung bình là 156,3 ± 19,8 g/l, Hct trung bình là 47,4 ± 5,6%, số lượng tiểu cầu trung bình là 208,95 ± 83,2 G/l, nồng độ CRP trung vị là 0,08 (0 - 0,34) mg/l. Có mối tương quan thuận mức độ chặt chẽ về số lượng bạch cầu, Hb, Hct và nồng độ CRP giữa máu cuống rốn với máu ngoại vi với hệ số tương quan r lần lượt là 0,7; 0,6; 0,6 và 0,5 (p < 0,01). Có mối tương quan thuận mức độ vừa của số lượng hồng cầu và tiểu cầu giữa máu cuống rốn và máu ngoại vi với r = 0,4 (p < 0,01).
Kết luận: Có mối tương quan thuận có ý nghĩa thống kê giữa số lượng bạch cầu, tiểu cầu, nồng độ Hb, Hct và nồng độ CRP của máu cuống rốn với máu tĩnh mạch ở trẻ sinh non bị NKSSS.
ABSTRACT
CORRELATION OF HEMATOLOGICAL PARAMETERS AND C - REACTIVE PROTEIN BETWEEN UMBILICAL BLOOD AND PERIPHERAL BLOOD SAMPLE IN PRETERM NEONATES WITH EARLY - ONSET SEPSIS
Objectives: Describe the characteristics of hematological parameters and concentration of C - reactive protein (CRP) of umbilical blood in preterm neonates with early - onset sepsis. Determine the correlation of hematological parameters and CRP concentration between umbilical blood and peripheral blood of preterm neonates with early - onset sepsis.
Methods: A prospective observational study. A total sample of 129 neonates under 37 weeks, were born at Hospital of University of Medicine and Pharmacy, Hue, Viet Nam from 6/2021 - 6/2022 and were collected umbilical blood for complete blood count and CRP after birth. Based on the risk factors of early - onset sepsis, clinical and laboratory follow - up for 48 hours, the divided into 2 groups: early - onset sepsis group and non early - onset sepsis group for comparison.
Results: In umbilical blood of 77 preterm neonates with early - onset sepsis, the median white blood cell count was 9,3 (8,1 - 11,8) G/l, the medium red blood cell count was 4,4 ± 0,7 T/l, the medium Hb concentration was 156,3 ± 19,8 g/l, the medium Hct concentration was 47,4 ± 5,6%, the medium platelet count was 208,9 ± 83,2 G/l. The median concentration of CRP was 0.08 (0 - 0.34) mg/l. There was a strong positive correlation between white blood cell count, Hb, Hct, and CRP between umbilical blood and peripheral blood with the correlation coefficient r respectively 0.7; 0.6; 0,6 and 0,5 (p < 0.01). There was a moderate positive correlation between red blood cell count and platelet count between umbilical blood and peripheral blood with r = 0.4 (p < 0.01).
Conclusion: There was a positive statistically significant correlation between white blood cell, platelet, Hb concentration, Hct, and CRP of umbilical blood with peripheral blood in early - onset sepsis in preterm neonates.
