RECONSTRUCTIVE PLASTIC SURGERY IN COMBINED TREATMENT OF PATIENTS WITH LOCALLY ADVANCED HEAD AND NECK TUMORS

Abstract

From 2008 to 2016 the Department of Head and Neck Tumors of the Tomsk Cancer Research Institute performed 120 reconstructive operations. The mostly patients had locally advanced tumor process: Т3 - 49; Т4 - 41. The localization of defects needed the reconstruction: oral cavity - 26 cases; tongue - 24 cases; skin (including defects of lower lip) - 12 cases; maxilla - 14 cases; larynx and hypopharynx - 12 cases; lips - 6 cases, cheek - 11 cases, mandibulla - 5 cases. For the reconstruction we used free flaps (83%) and pedicle flaps (17%). Good functional results were achieved in the mostly cases. Full flap necrosis was noted in 12 cases (10%). Fibular flaps necrosis was noted in 8 cases (7%). Thus reconstructive surgery promoted improvement of functional outcomes and quality of life of operated patients. Reconstructive phase of radical surgical treatment contributed full social rehabilitation of patients with head and neck tumors. To reduce postoperative complications when performing reconstructive surgery careful selection of reconstructive material was needed.
