Gastrointestinal Tract Damage in Systemic Connective Tissue Diseases (Juvenile Idiopathic Arthritis, Systemic Lupus Erythematosus) in Childhood

Abstract

В статье представлены данные о частоте и особенностях поражения желудочно-кишечного тракта (ЖКТ) при системных заболеваниях соединительной ткани, в частности при ювенильном идиопатическом артрите (ЮИА) и системной красной волчанке (СКВ). Большинство исследований, посвященных изучению факторов риска, диагностики и лечению поражения гастроинтестинальной системы, проведено на взрослой группе пациентов. В развитии поражения ЖКТ у детей с ревматическими заболеваниями ведущими патогенетическими факторами являются активность заболевания, наличие васкулита и проводимая терапия. Поражение ЖКТ может быть первичным, т. е. являться следствием аутоиммунного процесса, либо возникать вторично на фоне проводимой агрессивной противовоспалительной терапии. В обзоре указаны данные о связи ЮИА и сахарного диабета 1-го типа, а также СКВ и целиакии вследствие развития так называемого синдрома перекреста (overlap-синдром). The article presents data on the frequency and characteristics of damage of the gastrointestinal tract (GIT) in systemic diseases of the connective tissue, in particular in juvenile idiopathic arthritis (JIA) and systemic lupus erythematosus (SLE). Most of the studies devoted to the study of risk factors, diagnosis and treatment of lesions of the gastrointestinal system were carried out on an adult group of patients. In the development of gastrointestinal lesions in children with rheumatic diseases, the leading pathogenetic factors are the activity of the disease, the presence of vasculitis and ongoing therapy. The lesion of the gastrointestinal tract can be primary, i. e. be a consequence of an autoimmune process, or occur a second time against the background of aggressive anti-inflammatory therapy. The review provides data on the relationship between JIA and type 1 diabetes mellitus, as well as SLE and celiac disease due to the development of the overlap-syndrome.
