Management of skeletal Class III malocclusion is very important to avoid future surgical interventions. This case report demonstrates the successful treatment of skeletal Class III malocclusion in a 13-year-old male using a combination of Alt-RAMEC protocol and facemask therapy followed by fixed orthodontic treatment with extraction of 14, 24, 34 and 44. Post-treatment results showed significant improvements in skeletal, dental and soft-tissue parameters. The Alt-RAMEC protocol, combined with facemask therapy, facilitated maxillary protraction and provided effective skeletal modification. Early intervention in Class III malocclusion can yield favourable outcomes, emphasising the importance of comprehensive treatment planning and long-term follow-up.
