Low-Grade Appendiceal Mucinous Neoplasm with Obstruction of the Terminal Ileum

Abstract

Low-grade appendiceal mucinous neoplasm is the most common form of mucocele of the appendix arising from dilation of the appendiceal lumen by mucin. This is a case of a referred 43-year-old male patient who presented with a two-day history of sudden severe abdominal pain with low-grade fever, nausea, and vomiting. A diagnosis of ruptured acute appendicitis with peritonitis was made. He had exploratory laparotomy with right hemicolectomy performed. The histopathological examination of the sample sent was low-grade appendiceal mucinous neoplasm and hemorrhagic necrosis of the terminal ileum. Postoperation was uneventful, and patient was discharged to follow-up in the outpatient clinic. low-grade appendiceal mucinous neoplasm with obstruction of the terminal ileum is a rare presentation of an uncommon neoplasm. Laparotomy with right hemicolectomy has a good prognosis. Laparoscopy is discouraged for the fear of spillage of mucin, leading to pseudomyxoma peritonei, a life-threatening condition. The definitive diagnosis is on histological examination.
