A number of treatment options for Trevor’s disease based on the patient’s symptoms have been mentioned in the literature. An epiphyseal osteotomy, however, has not yet been covered in any publication report. We report a rare case of Trevor’s illness affecting the fifth metatarsal head, presenting as a painless fifth toe valgus deformity. At 1-year follow-up, treatment options such as osteochondroma excision and medial closing wedge epiphyseal osteotomy produced satisfactory clinical and radiological results. Trevor’s disease can be safely managed with the aforementioned approach, although it does require follow-up until the patient reaches the age of fusion of the epiphysis.
