Choroidal metastasis with retinal detachment as the first presenting sign of lung carcinoma in a female

Abstract

It is very unusual for lung carcinoma to present primarily with ocular features before respiratory symptoms. A 30-year-old female, with sudden onset painless diminution of vision in left eye (LE) of 1-week duration, was diagnosed with choroidal metastasis and retinal detachment. On imaging, she had lesions at multiple sites like lung and brain. Histopathology showed adenocarcinoma lung, followed by positive immunohistochemistry (Napsin-A and TTF-1) confirming primary adenocarcinoma lung. We managed with external beam radiotherapy and chemotherapy, resulting in vision improvement at 3 months. Thus, this case highlights a rare presentation of choroidal metastasis as the first clinical sign of adenocarcinoma lung in a female with successful outcome.
