Transarticular screw provides rigid C1 C2 fixation. It is associated with robust implant construct and high fusion rates. Combining sublaminar wiring provides rigid three point fixation. This technique can be safely considered in reducible AAD where a wide C2 isthmus along with a normal Vertebral artery course is present. Authors describe the technique in detail with indications, benefits over other similar procedures, short falls and detailed operative steps. “Pearls and Pitfalls” adds a fresh perspective to this less often considered procedure.
