Neurenteric cysts are one of the uncommon differentials for cystic lesions of the spine. Occurring most commonly in the cervical and cervicothoracic regions, purely lumbar neurenteric cysts are extremely rare entities. Here, we describe a case of a 25-year-old female patient who presented with complaints of chronic low back aches and difficulty in walking and underwent surgery. Histopathological examination revealed a neurenteric cyst. We also discuss relevant literature on the embryological, radiological, and clinicopathological aspects of purely lumbar neurenteric cysts.
