Secondary changes in thyroid diseases are very common. However, bone formation in thyroid associated with papillary microcarcinoma has been very rarely reported. Moreover, extramedullary hematopoiesis (EH) in thyroid malignancies is a rare finding and is commonly associated with hematological disorder. Here, we report a case of a 68-year-old female patient with thyroid swelling which was diagnosed as papillary microcarcinoma with osseous metaplasia, bone formation with EH. The patient underwent total thyroidectomy and clinical course of patient following resection was unremarkable. In this report, the patient with EH has no history of any hematological disorder.
