Although the incidence of renal malignancies has been on the rise, a broad spectrum of lesions, including hyperplastic, metaplastic, inflammatory, infectious and reactive, may occur in the urinary tract, which can mimic cancer and causing erroneous diagnoses. We present a rare case of chronic pyelonephritis with an inflammatory mass which mimicked urothelial carcinoma, which almost ended up in nephrectomy.
