Rescue Bridging Thoracic Endovascular Aortic Repair in a Case of Type III Endo-leak with Impending Descending Thoracic Aortic Aneurysm Rupture

Abstract

Abstract A thoracic aortic aneurysm (TAA) is a localized dilatation of the thoracic aorta more than 1.5 times the normal diameter. In view of rupture risk, it requires either open or endovascular treatment. Open repair has higher complication rates, hence, thoracic endovascular aortic repair (TEVAR) is considered the treatment of choice for TAAs. TEVAR is a relatively safer procedure but not free from complications. Endoleak is a known complication of TEVAR and can be life-threatening. Type III endoleak is caused by a defect in the graft either due to fabric tear or disconnection of modular overlap. It accounts for 2.1% of all endoleaks and can lead to sac rupture and death. Hence, monitoring of patients by computed tomography angio or contrast-enhanced ultrasound is advocated to diagnose it early. We hereby report a case of Type III endoleak with impending aneurysm rupture, managed successfully by bridging TEVAR.
