“Scarfrodesis”—Simultaneously Addressing Hallux Valgus and Hallux Rigidus With a Scarf Osteotomy and First Metatarsophalangeal Joint Arthrodesis: A Case Report

Abstract

This case report described a novel procedure used to address a concomitant hallux valgus and hallux rigidus in a patient in her sixties with rheumatoid arthritis. Initial radiographs confirmed concomitant severe hallux valgus deformity with first metatarsophalangeal joint osteoarthritis. A scarf osteotomy was performed followed by a first metatarsophalangeal joint arthrodesis in what the authors have called a “Scarfrodesis” procedure. At 1-, 2-, and 3-year follow-ups, the patient had complete resolution of pain, no limitation in mobility, and had complete consolidation of her arthrodesis. Her AOFAS Hallux Metatarsophalangeal-Interphalangeal Scale Score improved from 29 preoperatively to 72 at 1-year and 3-year follow-ups, with improvement in her Visual Analog Scale and SF-36 at 3-year follow-up. The authors advocate the use of this simple but elegant procedure, which has not been described in addressing the dual pathology in this cohort.
