18F-FDG PET/CT has been reported to be useful in differentiating tumor thrombus and bland thrombus. There are few reports on 68Ga-FAPI PET imaging features of tumor thrombus. Herein, we report a 46-year-old man with extensive tumor thrombus in the portal vein due to hepatic malignancy on 18F-FDG PET/CT and 68Ga-FAPI PET/MRI.
