Primary prostatic stromal sarcoma is extremely rare. Serum PSA is usually normal. Here, we report a case of primary prostatic stromal sarcoma in a 23-year-old man. 18F–prostate-specific membrane antigen (PSMA) PET/CT showed prostate mass and multiple low-density lesions in the liver with high PSMA expression. However, after chemotherapy, the level of PSMA expression in the prostate mass decreased, and PSMA expression lesions in the liver disappeared.
