Guidewires fracture may occur in severely calcified coronary chronic total occlusion (CTO). Rotational atherectomy (RA) is safe if the broken guidewire is firmly entrapped in the lesion. We present a case of using RA to open the calcified CTO and meanwhile grind the entrapped wire.
