Abstract

Abstract Background The study aims to describe the experience of grand multiparas in Japan who have been identified as specified expectant mothers to accept unexpected new pregnancies and decide to give birth and raise their newborns. The results of this study show potential implications for improvements in childbirth as a happy experience, safe and inclusive abortion care, a support for resolving health issues from financial difficulties to promote a financially stable life, or improvements in the quality of sexual and reproductive health and rights in Japan. Methods Specified expectant mothers visiting X hospital underwent a series of semi-structured interviews during their pregnancy. The interview entailed the living conditions of expectant mothers, the circumstances of their current pregnancy and the process of its acceptance, and their decisions or feelings regarding giving birth and raising their newborns. A phenomenological approach was employed as the analysis method. Results Participants included seven grand multiparas in their early 30s to early 40s, all of whom were expecting at least their fourth child. Three lived with their partners, and four were separated. Regarding their financial status, two of them received welfare support, and five of them were either employed or suspended/retired temporarily from their jobs. Three themes were identified as follows in the results of this study investigating the responses on unexpected pregnancies of grand multiparas identified as specified expectant mothers; 1) actual pregnancy under unintended circumstances with possibilities of pregnancy; 2) encouragement for delivery; and 3) live in relationship with someone irreplaceable. Conclusions The seven grand multiparas identified as specified expectant mothers had certain issues, including unmarried pregnancy, receiving public assistance, and financial difficulties from non-regular employment while raising three or more children. Despite these, they decided to give birth and raise the newly conceived newborns owing to the positive welcome from their partners or other children who appreciated their motherhood.
