Abstract

Abstract In the world of work, health, and safety (WHS) are essential. WHS is an effort to create a safe and healthy workplace to minimize the occurrence of workplace accidents. Companies in manufacturing have a lot of potential risks of work accidents. This research aims to identify the risk of work accidents in the process of manufacturing services. Identification is made that is looking for the highest value of work accident risk, using the FMEA method and looking for factors causing the risk of accidents. The results of the FMEA method carried out on the loading work of unloading cruid oil of PT Grand Kartech, Tbk. There were 45 potential workplace accidents. Potential work accidents that have the highest level of risk are that employees fall into the PIT, with a Risk Priority Number (RPN) value of 25. From these incidents, factors that can cause workplace accidents using the FTA method that has been used there are three factors: human, environmental, and material factors.
