Outcomes of the modified Brostrom procedure augmented with an InternalBrace and ankle arthroscopy for ATFL ruptures at Viet Duc University Hospital

Abstract

Tóm tắt Đặt vấn đề: Mô tả đặc điểm lâm sàng, chẩn đoán hình ảnh và đánh giá kết quả phẫu thuật tái tạo dây chằng sên mác trước theo kỹ thuật Brostrom cải tiến kèm gia cố InternalBrace và nội soi khớp cổ chân. Đối tượng và phương pháp nghiên cứu: Nghiên cứu lâm sàng mô tả cắt ngang tiến cứu 12 người bệnh được chẩn đoán tổn thương dây chằng sên mác trước và phẫu thuật tái tạo dây chằng sên mác trước tại Bệnh viện Hữu nghị Việt Đức từ 2019 đến 2021. Kết quả: Tất cả các trường hợp đều hài lòng kết quả phẫu thuật với kết quả tốt và rất tốt chiếm tỷ lệ 91,7%, điểm trung bình AOFAS cải thiện rõ rệt trước mổ là 63,6 ± 10,4 (55 - 70) so với sau mổ là 92,7 ± 4,47 (70 - 100), (P < 0,001) với thời gian theo dõi trung bình là 15,3 tháng. Tổn thương trong khớp thấy ở 11 khớp cổ chân (91,7%). Kết luận: Phẫu thuật tái tạo dây chằng sên mác trước kỹ thuật Brostrom cải tiến kèm gia cố InternalBrace và nội soi khớp cổ chân là một kỹ thuật hiệu quả, giúp làm vững khớp cổ chân, cải thiện kết quả lâm sàng đáng kể. Abstract Introduction: Describing the clinical and imaging features, outcomes of the modified Brostrom procedure with InternalBrace and ankle arthroscopy for ATFL ruptures. Patients and methods: 12 patients who suffered from ATFL tear and underwent surgical reconstruction. Cross-sectional study with longitudinal follow-up from 2019 to 2021 at Viet Duc University Hospital. Results: All of the patients are really satisfied with surgery results which have good and very good results for 91,7% and the AOFAS significantly improved from preoperative average 63,6 ± 10,4 (55 - 70) to 92,7 ± 4,47 (70 - 100), respectively (P < 0,001) with an average follow-up time of 15,3 months. Intra-articular lesions were found in 11 ankles (91,7%). Conclusion: Surgical reconstruction of ATFL and ankle arthroscopy are an effective technique, which helps stabilize the ankle joint, improve the clinical outcomes significantly.
