By performing bone marrow aspiration regardless of symptoms, these authors determined the prevalence of Leishmania species infection in a group of 291 HIV-positive persons in southern Spain. Amastigotes were found in 11% of patients; only 59% of the patients with amastigotes had symptoms attributable to visceral leishmaniasis. Persons with Centers for Disease Control and Prevention clinical category C were the most likely to have symptomatic visceral leishmaniasis. Symptomatic disease was also more prevalent in males than in females, and in intravenous drug users than in persons who were not intravenous drug users.
