N Engl J Med. 2012;366(22):2074–84 This article reports the Dutch CROSS trial, assessing the efficacy of neoadjuvant chemoradiotherapy (carboplatin/paclitaxel, 41.4Gy) followed by surgery vs surgery alone for locally advanced oesophageal cancer. Although fewer older patients were included, it represents a worldwide standard-of-care for trimodality therapy in oesophageal cancer. Thus, it remains highly relevant in any treatment plan for locally advanced disease.

There is less focus on age however side effect data and long-term remission rate with chemoradiotherapy +/- surgery—which may be a very important factor in the older population.
