Abstract

Acute disseminated encephalomyelitis (ADEM) is a demyelinating disorder presenting with myriad of neurological manifestations. We describe a case of a 22-year-old male presenting with bilateral lower motor neuron (LMN) facial nerve palsy with quadriparesis in a case of post Covisheild vaccination. T2-weighted Magnetic resonance imaging of brain revealed non-enhancing multifocal altered hyperintensity lesions, suggestive of ADEM. He was treated with methyl prednisolone and intravenous immunoglobulin, following which patient's clinical symptoms resolved. To conclude with, ADEM should be included in the differential in a patient presenting with bilateral LMN facial nerve palsy and upper motor neuron quadriparesis following COVID 19 vaccination.
