Telangiectatic osteosarcoma (TO) is one of the rare subtypes of osteosarcoma. As radiographic findings of TO are often confused with aneurysmal bone cysts, early tissue biopsy should be encouraged for diagnostic purposes. We report an original case of TO in a 28-year-old female presenting with severe pain and slight inability to bear weight on the right lower limb followed by a pathological fracture at the proximal right femur. It highlights the diagnostic and histopathological features of TO and discusses differential diagnostic considerations, treatment, and prognostic implications. The case was treated with hip disarticulation surgery along with neoadjuvant chemotherapy.
