Background and Aims
Previous studies have reported that 5-91% of patients with heart failure have thiamine deficiency. With the aging of heart failure patients, the number of heart failure patients with thiamine deficiency may increase. It is important to know the prevalence and clinical characteristics of heart failure patients with thiamine deficiency.
The purpose of our study is to investigate the prevalence and clinical characteristics of thiamine deficiency in patients with heart failure.
Methods and Results
The subjects were 256 cases who were admitted to our hospital for acute heart failure. All patients were classified into two groups, a thiamine deficient group (TD group N=72) and a normal thiamine group (NT group N=184), with the serum vitamin 24 ng / ml, which is the standard value in Japan, as the cutoff value. The clinical features and outcomes of both groups were investigated. The endpoint was set to one-year of cardiac death and readmission due to heart failure.
Of all 256 cases, 72 (28.1%) had thiamine deficiency.
The TD group had a significantly higher incidence of one-year cardiac death (25.0% VS 13.0% P=0.02) and readmission due to heart failure (38.9% VS 23.9% P=0.01)
Multivariate logistic regression analysis revealed that serum thiamine levels was relative factors in the incidence of readmission due to heart failure.
Conclusion
Thiamine deficiency is not uncommon in patients with heart failure.
Patients with heart failure who have thiamine deficiency have a high incidence of readmission due to heart failure, so treatment aimed at preventing readmission is necessary.
