We describe a case of an adult patient with a left temporal hematoma due to a dural arteriovenous fistula (DAVF) of the pterion. The left middle meningeal artery (MMA) was the major feeding artery of the DAVF and was also supporting the territory of the ipsilateral posterior inferior cerebellar artery (PICA). This is first case described in the literature of a PICA arising from the MMA. Accurate knowledge of vascular anatomy and its variations is essential for the endovascular treatment of cerebrovascular diseases. A complete diagnostic angiography is crucial to plan the correct endovascular strategy and to minimize the risk of complications.
