The collision of pituitary adenoma and craniopharyngioma has rarely been reported. This study describes a 50-year-old woman who presenting with symptoms of extremely fatigue and blurred vision. Magnetic resonance imaging (MRI) showed a large enhancing sellar mass with suprasellar extension that was 14×15×16 mm in size and compressing the optic chiasm. This patient underwent transsphenoidal surgery with gross total resection and the pathologic and immunohistochemical examinations showed pituitary adenoma with craniopharyngioma. During a follow-up period of two years, MRIs identified the recurrence of the lesion and the patient requires reoperation.
